ABSTRACT

Objective To compare the prevalence of HIV/STD and related health care seeking behaviors among male STD clinic attendees between Xi'an and Xianyang cities.Methods During June and July 2016,206 male STD clinic attendees were studied in Xi'an city,with another 221 male STD clinic attendees in Xianyang city.Cross-sectional questionnaire survey was used to collect attendees' behavioral information.Blood samples were collected via HIV/HCV/Syphilis testing.Results The prevalence rate of HIV infection was 2.4% (5/206) in Xi'an and 0.9% (2/221) in Xianyang,with no statistical significant difference between the two cities.The prevalence rate of syphilis was 4.9% (10/206) in Xi'an,which was significantly lower than 13.6% (30/221) in Xianyang.The proportion of respondents,diagnosed with other sexually transmitted diseases,in Xi'an was higher than that of Xianyang.The proportions of commercial heterosexual sex and sex with temporary sexual partners in the past 3 months were 18.0% (37/206) and 15.5% (32/206) in Xi'an,lower than 46.6% (103/221) and 15.8% (35/221) in Xianyang (x2 =39.70,P <0.01;x2 =-0.01,P=0.93).The proportions of condom use with commercial sex workers or temporary sexual partners in the past 3 months among Xi'an were 37.8% (14/37) and 6.3% (2/32),lower than 93.1% (95/102)and 57.1% (20/35) in Xianyang (x2=49.06,P<0.01;x2=19.63,P<0.01).Conclusion Differences were noticed between Xi'an and Xianyang city in terms of STD and HIV prevalences,behaviors related to commercial sex and use of condoms among the male STD clinic attendees that calling for targeted actions in control of high risk behaviors in both HIV/AIDS and STDs transmission.

ABSTRACT

Objective To observe the neural mechanism of learning and memory network disturbance induced by 36 hours sleep deprivation(SD).Methods 18 young male healthy volunteers were enrolled in this study.Resting state functional magnetic imaging(rs-fMRI) data was collected by blood oxygen level dependence functional magnetic imaging(BOLD-fMRI) during rest wakefulness(RW) and 36 hours SD.Bilateral hippocampus were selected as seed points by REST software automated anatomical labeling template.And then compared the differences of functional connectivity(FC) between bilateral hippocampus and whole brain regions by using paired-t test within statistical parametric mapping 8 (SPM8) software.Finally,correlation analysis had been done between significant FC of brain regions and WMS MQ values (SD-RW).Results Compared with RW,the bilateral hippocampus showed positive connected with the right dorsal posterior cingulate cortex,and showed negative functional connectivity with bilateral postcentral gyrus.The WMS MQ was significantly decreased after 36 hours SD.The connection between bilateral hippocampus and the right posterior cingulate cortex showed negative correlation with the deductionofWMS MQ (left:r=-0.463,P=0.015;right:r=-0.546,P=0.003).Conclusion SD has a significant effect on bilateral hippocampal FC.The FC between bilateral hippocampus and right dorsal posterior cingulate cortexis significant increased,while between bilateral postcentral gyrus is decreased after 36 hours SD.The former may be an important neurological mechanism in learning and memory ability regulation after 36 hours sleep deprivation.

ABSTRACT

Objective To design and synthesize novel drugs for metabolic syndrome. Methods A kind of drugs treating met?abolic syndrome was designed by linking acipimox with the lipid lowering function group of fibrates. Primary amine intermediates were synthesized from 4-aminophenol,4-(aminomethyl)phenol and 4-(2-aminoethyl)phenol by 3 steps,then target compounds were ob?tained by coupling acipimox with these primary amine intermediates,and their hypolipidaemic activity in Triton WR-1339 induced hy?perlipi daemic mice was evaluated. Results and Conclusion 5 target compounds were synthesized and identified by 1H NMR and ESI MS methods. It showed that all the compounds could decrease blood-lipid,and 4c exhibited anti-hyperlipidemic activities close to the positive control(bezafibrate)in in vivo hypolipidemic activity tests. The results have good value for the discovery of novel drugs for metabolic myndrome.

ABSTRACT

Objective@#To investigate the value of preoperative dynamic contrast-enhanced MRI in reducing the rate of tumor-positive resection margins after breast conserving surgery in patients with early non-mass breast carcinoma.@*Methods@#Seventy-two patients with early non-mass breast carcinoma received ultrasonographic and mammographic examination and subsequently underwent dynamic contrast-enhanced MRI examination before breast conserving surgery. The control group consisted of 74 patients who had early non-mass breast carcinoma. They only received ultrasonographic and mammographic examination and didn′t undergo contrast-enhanced MRI examination. The comparison of the rate of tumor-positive resection margins between two groups was performed. The MRI findings that had the significant influence on the rate of tumor-positive resection margins were analyzed using Logistic regression model.@*Results@#In 28 patients (28/72, 38.9%), dynamic contrast-enhanced MRI could correct or supplement the ultrasonographic and mammographic findings and resulted in the reasonable change of surgical program. The preoperative MRI examination group (n=30) had lower rate of tumor-positive resection margins than control group for invasive ductal carcinoma (23.3% vs 40.0%, P=0.02), but there was no significant difference (21.4% vs 26.9%, P=0.10) between two groups for ductal carcinoma in situ (n=28). The preoperative MRI examination group (n=14) had lower rate of tumor-positive resection margins than control group for the other pathologic types of breast carcinoma (14.3% vs 38.9%, P=0.02). The statistical analysis on the basis of Logistic regression model showed that some main MRI findings, including change surrounding the tumor, distance between tumor and nipple and tumor size, had the significant influence on the rate of tumor-positive resection margins.@*Conclusion@#Preoperative dynamic contrast-enhanced MRI significantly increased the accuracy of resection margins evaluation, and greatly reduced the rate of tumor-positive resection margins after breast conserving surgery in patients with early non-mass breast carcinoma.

ABSTRACT

Objective To investigate the feasibility and value of CT perfusion imaging in combination of CTA 320-slice CT with dual-low-dose in detection of acute cerebral ischemia.Methods Forty patients with acute stroke underwent perfusion imaging and CTA using a single scan with 320-slice CT with dual-low-dose schedule.CTA,4D-CTA,4D-perfusion imaging and fusion images were generated at a post-processing workstation.All patients also underwent a 3.0T magnetic resonance imaging with DWI for a comparison.Quality of the images,degree of vascular stenosis,and location of ischemic lesions were evaluated.Results The vascu-lar stenosis or occlusion was found in 33 patients,8 of whom were confirmed by the DSA.The ratio of better images of CTA was 82.5%.CTP showed 297 lesions with abnormal perfusion.202 lesions were confirmed as infarct by DWI,while 95 ones were nor-mal on DWI but with increased DLY and TTP and slightly decreased CBF and CBV in 49,increased DLY and TTP and normal CBF and CBV in 21,and increased DLY and TTP and slightly increased CBF and CBV in 25.Conclusion CT perfusion in combination with CTA by 320-slice CT with dual-low-dose is feasible and useful to observe accurately the vascular structures and the ischemic status of the whole brain at early stage.

ABSTRACT

PurposeTo investigate the feasibility and clinical value of 3.0T MRI iterative decomposition of water and fat with echo asymmetry and least-squares estimation (IDEAL) technique in imaging normal lumbosacral nerve roots.Materials and Methods Twenty-five healthy volunteers and 15 patients with lumbosacral pain were examined on MR scanner using IDEAL sequence thin-layer coronal scanning and reconstruction technique. The evaluation of bilateral lumbosacral nerve roots from L1 to S1 included the detection rate of lumbosacral nerve roots, the length of preganglionic nerve, the width and length of dorsal root ganglia (DRG) and the length of nerve root sheath.Results The anatomic structure and the contour of spinal nerve roots were well demonstrated on IDEAL sequence imaging, with 100% detection rate for preganglionic nerve and DRGs of nerves from L1 to S1 level, as well as the structures posterior to the ganglions from L3 to S1 level. The structures posterior to the ganglions at L1 to L2 level were shown as grade I in 12% and 42%, grade II in 16% and 32%, grade III in 72% and 26%. The length of preganglionic nerve, DRGs and nerve root sheath from L1 to S1 were gradually increased (P<0.05 orP<0.001). Normal lumbosacral nerve roots appeared as isointense linear structure in the nerve canals.ConclusionIDEAL sequence can depict the anatomic structure of the lumbosacral nerve roots and provide reference standards for lumbosacral nerve roots compression.

ABSTRACT

Objective To improve the synthetic condition of 6‐methoxyisatin .Methods The starting material 3‐me‐thoxyaniline was firstly converted into the intermediate 1‐oximino‐N‐ (3‐methoxyphenyl) acetamide through Sandmeyer reac‐tion ,then 6‐methoxyisatin was conducted with methanesulfonic acid as catalyst in the following ring closure reaction .Results In the second step of the cyclization reaction ,methanesulfonic acid was used to replace the concentrated sulfuric acid which is widely used in literature .The temperature was 80℃ ,the reaction time was 30 min ,and the yield was 81.24% .Conclusion This study provided a synthesis process of 6‐methoxyisatin with simple operation ,mild reaction condition and high yield ,which is suitable for large scale preparation of the compound .

ABSTRACT

Purpose To evaluate the diagnostic significance of non-contrast enhanced magnetic resonance angiography (NCE-MRA) for lower extremity arterial stenosis on a 3.0T MR scanner, in order to provide a reliable method for clinical application. Materials and Methods Thirty patients with arterial disease in lower extremity underwent NCE-MRA before contrast enhanced magnetic resonance angiography (CE-MRA). Image quality of the two methods was compared. The diagnostic accuracy for significant stenosis ( ≥50%) of NCE-MRA was assessed using CE-MRA as a golden standard. The consistency of the two methods in diagnosis of significant stenosis ( ≥ 50%) was analyzed. Results All patients successfully underwent both NCE-MRA and CE-MRA examination. There were 532 arterial segments detected by NCE-MRA. In the calf region, venous artifacts presented more frequently on CE-MRA (Z=4.92, P<0.01), while in the abdominal and the femoral regions, venous artifacts presented more frequently on NCE-MRA (Z=4.58 and 3.56, P<0.01). The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of NCE-MRA for the diagnosis of significant stenosis ( ≥ 50%) were 97.89%, 97.69%, 97.74%, 93.92% and 99.22%, respectively. There was good agreement (Kappa=0.94, P<0.05) between the two methods. Conclusion For the imaging of lower extremity arterial stenosis, NCE-MRA shows similar image quality and diagnostic accuracy with CE-MRA, thus can be used as an alternative method for lower extremity arterial stenosis in patients who have renal insufficiency or other contraindication of contrast media.

ABSTRACT

Objective To explore the value of PET-CT and CT in the assessment after radiofrequency ablation (RFA) in lung cancer.Methods Eighteen patients were randomly divided into short-term group ( 8 cases) and long-term group ( 10 cases).Patients in short-term group received PET-CT and CT examination within 10 days after RFA therapy,whereas those in long-term group got PET-CT and CT examination within 2 months after RFA therapy.The value of PET-CT and CT after RFA therapy through the result of the image was compared between two groups.Results All patients were classified as stable disease in CT,while in PET-CT assessment,33.33% (6/18) of the patients presented complete remission and 66.67% (12/18)presented partial remission.37.50% (3/8) of short-term group presented complete remission,62.50% (5/8) of short-term group presented partial remission.30.00% (3/10) of long-term group presented complete remission,70.00%(7/l0) of long-term group presented partial remission.Conclusion PET-CT is superior to CT in the assessment of the effectiveness of RFA therapy,and there is no difference between short-term and long-term PET-CT examination,therefore early PET-CT is more with clinical significance.

ABSTRACT

Objective To evaluate the effieaey of test bolus technique with multi-slice spiral CT (MSCT) for determining the optimal scan delay time in CT Hepatic artery (HA)-portal vein (PV) angiography and multi-phase scanning.Methods MSCT liver angiography and multi-phase scanning were performed in 187 patients divided randomly into two groups.In group A (n =59),the scan delay time was set according to the subjective experiences of operators; in group B (n=128),the scan delay time was determined by test bolus technique.Abdominal aorta and superior mesenteric,vein were selected as target blood vessels,and 50 HU was set as enhancement threshold value.20 ml contrast agent was injected intravenously and time-density curve of target blood vessels were obtained,then HA-PV scanning delay time were calculated respectively.The quality of CTA images obtained by using these 2 methods were compared and statistically analysed using Chi-square criterion.Resuits For hepatic artery phase,the images of group A are:excellent in 34(58%),good in 17(29%),and poor in 8 (13%),while those of group B are excellent in 128( 100%),good in 0(0%),and poor in 0(0%).For portal vein phase,the images of group Aare:excellent in 23(39%),good in 27(46%),and poor in 9(15%),while those of group B are excellent in 96 (75%),good in 28 (22%),and poor in 4 (3%) respectively.There was statistically significant difference between the ratios of image quality in group A and group B (X2=14.97,9.18,P < 0.05).Conclusion Accurate scan delay time was best determined by using test bolus technique,which can improve the image quality of liver angingraphy and multi-phase scanning.

ABSTRACT

Objective To study the clinical significance of using MSCT(16 multi slice spiral CT,MSCT)to evaluate the patency of coronary arterial stent,and to explore the factors contributing to the image quality.Methods 32 patients with coronary arterial stent im- plantation received MSCT.The shape and position of the stent and the situation of restenosis were checked with several methods.Of the 32 patients,the results in 7 cases were further verified by X-ray coronary artery angiography.The image qualities were classified in three grades.The influence factors contributing to imaging quality were analyzed.Results The rate of good image quality for MSCT coronary artery angiography was 87.5%(28/32).The rate of good image quality for the patients with heart rate less than 60 was 94.7%,for those with heart rate of 61-70 was 88.9%,and for those with heart rate over 70 was 50.0%,there existed significant differences(X~2=16.354, P

ABSTRACT

Objective To evaluate the clinical application of spiral CT pulmonary angiography(SCTPA) in the diagnosis of pulmonary embolism(PE) and its limitation.Methods 52 cases of PE were analyzed.SCTPA were performed in all cases with slice thickness 3 mm,pitch 1.5~2.0,scanning time 0.8 s,reconstruction thickness 1mm.The reconstructions of SCTPA included MIP,MPR and SSD.Results 2898 pulmonary arteries in 52 cases were observed.The direct manifestations of PE were showed in 927 pulmonary arteries(32.0%) by SCTPA,in which superior segmental and inferior segmental of pulmonary arteries were involved respectively.The indirect manifestations of PE were showed by plain scan in 113 cases.Conclusion SCTPA is a high effective and noninvasive method in the diagnosis of PE especially for the PE of superior segmental.

ABSTRACT

Objective To investigate the value of multislice spiral CT in evaluating the parametrial invasion of cervical carcinoma. Methods 75 cases of FIGO staging II cervical carcinoma who had received the surgery were studied. Their imaging and the FIGO staging were analyzed retrospectively. Results For FIGO staging, the accuracy for evaluating the parametrial invasion was 53.3%(40/75), the rate of underestimation was 26.7%(20/75), the rate of overestimation was 20.0%(15/75); For CT, the accuracy of evaluating the parametrial invasion was 69.3%(52/75), the rate of underestimation was 14.7%(11/75), the rate of overestimation was 17.3%(13/75). In 40 cases, both FIGO staging and evaluation by CT were consistent with the surgical results. In 35 cases, FIGO staging was different from the surgical results. In 29 cases, evaluation by CT was different from the surgical results. In 10 cases, CT could correct the error of FIGO staging. Conclusion CT scan can acquire the multi-plane image, and can directly reflect the parametrial space. It is more accurate than FIGO staging, and is an important complement to FIGO staging.
